actually i went back from the hospital around 3 pm... when i reached home... terus collapsed. pnat kot... if not.. mybe sbab morning bgun awal dr usual.. oww yaa... i forgot to tell in the last entry... we (the SPM medical students) were required to reach at the hospital at 6.30 am this morning. so i need to be very punctual n "immune" to this kinda stuff.
now its like 7.45pm... goin to start typing, k~
as early as 6am in the morning, (still dark and scary), i thought i was the first person to arrived... but still, there's more "lagi gila awal" dari loner... itu 2 budak Serian meh, Flixo n Josephine... nnti kt bawah i tell which one is who... after waiting at the lobby for about half an hour (not only wait laa, we sempat take picture n some of us bole breakfast lagi... ), we were called by Matron Fu to the "bilik mesyuarat utama", or the Main Meeting Room~ :P
She started our day wif some motivation so we realised that though the job is that crazy, n salary also not lumayan, we still have some more reason to adore the medicine line in this life. Yes, she was very good in motivating... veee~ry the convincing... ahahha... seriously meh... memang... terangkat bulu roma, membuak-buak perasaan nk jd doktor... she did it so naturally... i mula2 x perasan pun yg Matron tengah bagi motivation... until she said "i was asked to motivate you by the pegawai atasan every day before u started the 'job' again" .. lol... 5 stars Matron Fu!!
besides giving some kind of lecture to us, this was also mentioned by her... the 3 important criteria whenever u work wif Ministry Of Health Malaysia (KKM)... i believe theres goin to be more of us going to serve for KKM n i should share... u said why?? because i care loo...

for us (34 of us in the program), she said, we are the cream among the cream, so we have to be the creamy type among the societies.. And the doctor is really rewarding job (not totally in term of salary), so like my friend said to me, take all the hardest thing first so at the end of the tunnel, its a bright life.. same as wat the matron said,"take out everything as a challenge, life is not that smooth". yeaa... everything was perfectly fine for the first hour of the morning till she said, "hand in report in a week time!".. luckily i jot down everything in my blog, so i can refer time to time.

theres something new this morning since we came early, the three of us were "lucky" enough to join the 'HANDOVER session'... it was a crowded room... (very small 1 to have 40 plus Doctor in it). i was only manage to stand next to the white screen (xray viewer la senang cerita).. one doctor will stand in front (which is next to me) and doing her talking, while the other one showing xray film... the one who is talking is the doctor ON CALL last night.. so, anything new during on call, she'll tell the other H.Os, M.Os and also the specialists in ortho. from wat i heard, most of the cases were caused by accident, majority of the accidents were by the 'lumba haram' thing... as a doctor, we have to treat. After puas berfikir solving problems, suggestions, the 45 minutes session ended. the room was very very very hot i tell u. i was sweating all over. before dismiss, one of the specialists called us and im very glad to introduce myself and talked to him.. excited nya dapat cakap dengan specialists... bebudak lain, ada jgak yg x prnah jmpa pun... then sesi macam yesterday started.. the AM ROUND.. (yesterday PM ROUND).. comparing wif yesterday 1, today was more detailed n more doctor are coming.. maybe because its in the morning so the AM ROUND and PM ROUND would be slightly different. the most outstanding H.O i observed from yesterday is DR NOOR IZATI... waa.. very committed!!
we only went to some patients because i think, the patient we went for ROUND session were those who were discussed during the HANDOVER session. More to checking and planning for operation or to discharge (future action). Still the M.Os need to check on what the ate, any poo, how often the patients walk and move by the HISTORY TELLING method. (theres 3 methods to identify problems on patients, i tell u later k~)
Besides that, the doctors also talk to the family to cheer and to comfort them up. we went to beds located in front of the nurse table which is called ACUTE BEDS... ok, an acute bed is a bed, where patients lying on it requires immediate action... like dr. told me,,, "we placed 'collapsed anytime patients' here".. theres two critical patients... 1 i suffering from diabetes where she need to say "Yes" to the doctors before they cut her three toes.. if she delayed the operation... it might merebak to the whole feet and during that time, we need to cut it off... (waa,,, 'we'?? dah ambik mood doctor la ni).. she didnt said 'yes' maybe tension kali... i pun, kalau dh atas bed tu,, nk fikir pun susah.. apa lagi nk buat decision to cut toes~ (orthopaedic ward is not only locating patients wif bone dissorder, or fraction. but as long as ada kna mengena dgan kaki dan tangan, msok la ward ortho. some Hospital send diabetes patient to Surgery Ward, but in SGH, its ortho)
the other 1 is a male patient... also diabetes but thats the minor problem... i didnt catch wat the dr said but after explanation, both the kidneys of the patients sudah rosak... aiyoo.. dialysis laa... we learn that in Biology SPM... not only that, the best part is, the patient allowed us to touch his hand where the ARTERIOVENOUS FISTULA (AV Fistula) were located. its a connection between the artery and vein for dialysis. i can feel the blood passing through the av fistula... how to say hoo... its something sound like this laa; "shaa shaa shaa.....".. ahaha... just want to give uols readers time to imagine... credit to DR NG INN SHAN. (H.O)
and for other patients, still the same... i sempat sneak in nk tengok wat the DRs wrote... (tulisan mseh cantek oo). theres the name of DRs in charge for the patient, observation on the patient including amount of poo or any nausea, ability to walk etc.. and also written on plan of action... this is the time where the H.Os n M.Os will discussed among each others (during the AM ROUND and same as PM ROUND later).
then 1 Dr tarek me out of the group of other Drs... he asked 'u understand???' ahaha.. i said "yes"... waa.. so good, he sindir me... then i said "no..", and he laughed... he want me to follow him and i just follow laa...

so, we went to the lab... where he needs to send the blood for blood test.... sambil2 jalan, he teach me a lot laa... the most important part is the 3 methods to investigate the patients' problems.
1. HISTORY TAKING - where DRs goin to ask patients a lot of things, including family history, prnah sakit ke, jatuh ke, accident ke... but sometimes, if the patient xsdar, the family member will tell laa... this case, sometimes we call it HETEROANAMNESIS
2. PHYSICAL EXAMINATION - the DRs need to touch the patient.. most of the time, u cannot only see...
3. INVESTIGATIOAN - this 1 is wat we just did... i mean partly.. we sent the blood for investigation. besides blood, urine also can, saliva... it depends on wats disease we expect. tp, kalau AIDS, memang kne ambik darah~
dr said, if u master all these skills, u can become a great doctor.. sounds easy huh???

(kenyang nasik beriyani sudah~~). after chit chat juicy2 thing (Dr, pn ada masa ok), i was sent back to the Female Ortho ward... our group start to discuss watever we had leared in this 2 days... tapi 50% borak. lepas dont know wat to do, we searched for Dr. Syaz (M.O), aaaandd,,, she's on call arini... bertuah sungguh... she was downstairs at level 1 in ANE.. (or ground floor, not sure).. lepas pusing2 dlm dat area, we found her... she let us follow her everywhere (luckily she didnt went to the ladies)...
the place was busier than the ortho ward. because people come n go, come n go~ firstly *they need to check at the ANE whether *they need to go to ward or just check up and follow up wat the DRs said.. (*they refer to patients)
smpat pulak lihat drama M.Os bengang dngan junior (H.O).. fuiyoo. ganas.. muka ayu, tapi perggh!!! respect sungguh... we follow like 4 cases, one of them is a 3-year old kid.. from wat i understand, he (the child) was sent to a hospital but the fracture on his tibia n fibula was not detected... (misslook , careless, we dont know)... we went to the clinic, but waited outside... dont know wat happened inside. then another case, swollen leg... given half P.O.P, because of fracture also... another lebih berumur man,,, look very healthy.. can move both his hand freely... but after xray, the carpals was extremely away from the original position. n we sempat la interview patient ni.. yes... we... budak2 ni.. Dr2 tgh sibuk buat preparation to betolkan balek.. he met an accident this early morning, thats all.. (bawah lagi, i show u pic)
then we went for lunch.. our group is the last group to had lunch... busy sgt kot~ ahaha... that afternoon... so excited!! MORTUARY ROOM is waiting for us... in our head was like, waa.. the mayat is waiting oo.... sangat teruja!! but then so frustrated... y??

4. bnyak equipment n refrigerator rosak
5. paling mem'frust'kan, NO mayat!!!
so sad.. sangat sad... my phone was ready to take the d pic of dead body but, theres none of them. its ok, we still learn something, the refr must not below freezing temp, only 2 - 8 dgree celsius... so, its easier for post mortem session.. the officer gave example of a fish, if let freeze, its hard to masak.. masak syowang2!! oklaa....then around 3 sudah bole balik... n dats all for today~ in short, the AM session is better than the PM.

This is how they work..everytime the ward received patient, the H.O will discussed the case wif the M.Os... also need to refer to specialists. about the amount and dos for medication, the DRs will refer to the pharmacist... hello future pharmacist!!! u work in ward too ok... jgn nk lepak2 dlam lab or pharmacy jee...

Method no. 2, PHYSICAL EXAMINATION. DR Syaz is examing the swollen part near the elbow of the man... its not fluid, its hard... not nanah also... and, this patient was sent to the other clinics for further check up.. if the swollen part increase in size, he needs to meet DR Syaz n the geng again...

This is the "bed" for the dead body,,, the pillow is as hard as a kayu... memang kayu pun... below, are the alatan2.. wpun officer tu x bnarkan amik gambar.. ooopss.. x sengaja ok~ Then we went back home... lagi tired dari semalam!!! tp, 3 times lagi best!!!! tomorrow, we all are promised to be brought to Pusat Jantung Hospital Umum Sarawak (PJHUS) at Samarahan... hopefully not frustrating laa because Matron Fu said, the place look like a resort.. really??? never been there before... ok, thats all for now.. bbye~
